Aviva Life Insurance
Not providing Employees PF Form for withdrawal

Hi,

I had worked with Aviva Life Ins. company in the year Dec.2007 to 2009 Feb. Once I left the company giving formal resign, the on board manager of mine did some cunningness I don't what he had communicated to HR

but I have with me the resignation letter. Now my PF is stuck with the company and they are not providing me the form to withdraw it. I already had sent number of mails to HR but no response. My Employ ID is: 11265 and my PF No. is: DL/CPM 0025808/000/0011194

Kindly inter-vein and help me in getting my money.
