UNDER RTI ACT
I am the holder of gas connection no 100191 of chavi gas agency roorkee & I need following information under RTI ACT
1. My home address has been changed in DEC . making many repeated requested in writing along with the photo copy of govt. allotment letter my home address is not being changed till date . tell me the circumstances under which my home address is not being changed till date.
2 what action will be taken by you against the employee who is responsible for this negligence.
3. what is the actual weight & actual cost of the cylinder.
4. whether the cost of home delivery is included in the amount received by you by the customer.
5. Please provide the copy of the orders under which you are booking the gas after 22 days from the delivery of cylinder.

Sir,
I have post the RTI application along with the Postal order through registered post having no. A 2779 to the information officer of CHAVI GAS AGENCY BSM CHOWK Roorkee on date 17/04. But the CHAVI GAS AGENCY BSM CHOWK Roorkee denied to receive my application with the remark “ lene se mana kar diya”
The said refunded application I received on date 19/04 . This is why Ihave to approach to higher authorities.
