Unable to avtivate jet prilage card

I was asked to fill the form for privilege card by. airhostess. Also was alloted a temprory card the no is 143658023.

This is 3 months old, after that traveled 3-4 times but till now the card is not activated.

Pls do the needfull and revert back on the same.

Also acknowledge for the same.
