MTS Broadband connection(MBLAZE)

I have brought prepaid MTS Internet connection through agent (MANOJ KUMAR) Phone # (9133149995, 9133036397 not picking call). I have submitted the Driving licence as address proof (OUT SIDE from Jharkhand). After using a few week they disconnected the account. I was talk customer care number, they told me submitted the document again i did that after that my Internet services resume for two day's Then again they disconnected. Again i talk to customer care number they told me we can not resume yur service beacuse of yur connection disconnected permanently.

Please help me to activate the service again..
