No information of completing my 6 months package

I want to disconnetct my broadband connection.bcoz i have completed my work but we have no information for complte my 6months package.

Now they will show outstanding so i cant pay it.please disconnect my connection otherwise they will increase my outstanding and i will not abale to pay bill.
