Non issue of ATM application

I am an account holder in your Imphal Branch, Manipur State for the last 20-25 years. I had visitewd your Imphal office to obtain ATM application form but I was told that they have stopped issuing ATM application forms.

Kindly let me know whether you have stopped issuing ATM application forms or not. The attitude of your responsible staff will drive away customers to other customer friendly banks.
