I have applied for shifting of my telephone no.2471 0661 from 12 D/18, Naktala Lane, Kolkata 700047 to premises no. 10P, D.P.P. Road, 1st Floor, Kolkata 700047 almost a week back. I have carried the telephone instrument myself but did not carry the connection box since it was broken. In the circumstances I request you to immediately give the connection at my new address and supply a connection box for which I shall pay the necessary charges if required. You're immediate action is solicited.
