Not receive pan card

Sir,

most humbly i beg to stay that i (MADHU GHOSH) applay a pan card (acknow.no.-013670100006990, ac date-22/11/07 and PAN NO.-AMUPG1707A) from Tin Facilitation and PAN Centre managed by NSDL, TIN-FC ID :01367, Ashiana trade Centre, Adityapur, Jamshedpur-831013.

But cover a one year i do not received my pan card till now.
