Pan card is not come after four month

Espected sir,

i am, nimitkumar mahendrabhai patel, from olpad, surat. i fill up form of pan card and submit in surat pan office.i also recieved my pan card.

but in pan card my date of birth is wrong so, i submit pan card in pan office and give process.

after five month i have not pan card. pan office has been not provide pan card.

so, i request you to that my pan card is provided by pan off mumbai very early.
