Sir,
I have shifted to Lucknow from New Delhi in the month of October and currently I am residing at the following address, with my family.
46-A, AWHO colony, Tyagi Vihar, Lucknow 26002.
I have submitted duly completed form-6, along with photographs and connected certificates in the month of October for issue of voter identity cards, for myself, my mother (Mrs. Kaushlya Mehra), my wife (Geeta Mehra), and my daughters (Pallavi Mehra and Mridu Mehra).
I have not recieved voter identity cards till date.
It is requested necessary direction may please be issued to the connected staff for issue of the voter cards to me and my family members.
