Need a CDF form

Hi, this is Harshit Shah frm Ahmedabad... my citi bank credit card had been stolen by someone and it was misused by the same person... so i am not able to pay my outstanding as it was made by someone... you are requesed to send me a CDF (Customer Declaration Form) so i can submit the same to you and the investigation can be implemented... please send me CDF form on below mentioned address:-
