Wrong entries in my voter Identity Card

In my Voter Identity Card (XBO0432526) the following wrongs observed please rectify:

1. My name is wrongly entered as K V Vijaya Laxmi instead of K V S T Vijaya Lakshmi

2. My Husband name is shown as suryanarayana instead of K V V Suryanarayana

3. My Date of Birth is wrongly shown as 1974. Instead of 08/08/1973.

4. In the Address column shown as Pallavipark instead of Ashokapark.
