Corrections In EPIC card for General Elections 2009

Dear CEO Andhra Pradesh,

I have the following details wrongly entered in the voter card issued by you for general elections 2009 in andhra pradesh.

My House No is wrongly printed as 2-60/1 instead of 2-47/1

My Date Of Birth is wrongly printed as XX/XX/1988 instead of 8/8/1984

My Father's Name is wrongly printed as Darma rao instead of Dharmarao

Please consider the details for the correction and issue me a new EPIC card at the earliest possible.
