Wrong address and mis spelt names on Voters ID card, and non inclusion of names of family members in the voters list

Sub: Regarding discrepancies in new votes and wrong data printed on Voters ID cards.

Respected Sir,

This is to bring to your kind notice that I, Dr. Vivian Haque, age 65 yrs am a resident of H.No-1, Nikka Singh Colony, Ram Tirath Road, Amritsar, Punjab-143002. I reside with my family consisting of my Wife, Smt. Premlata Haque, age 63 yrs, my son, Navin Haque, age 35 yrs, and my daughter-in-law, Prasannta Haque, age 34 yrs. All four of us had applied for new votes in Dec and for our names to be included in the voters list of Part-54, Municipal Ward -40, Amritsar 16-(W) constituency, in duly filled application forms in Form No-6, accompanied by the copy of ration card as the residential proof. We were assured by the person on duty who was receiving the forms, that our names will be included in the new list. The names were published in the new list but the addresses were wrong, moreover no voter ID cards were made at that time. When it came to our notice, we applied for change of address in Form no-8, again accompanied by the copy of ration card as the residential proof. This was done at the district office of the election commissioner, which is the Office of the Excise and Taxation Commissioner, Dist-I, Amritsar, Punjab. The forms were received on 14-7-08, by the staff under the supervision of Mr. Rajinder, who was the in-charge of this affair. (We have the signatures of the receiving official on the acknowledgement on both occasions).

In Feb the booth level officer of Part 54, Municipal Ward -40, Amritsar 16-(W) constituency, Mr. Kulwant Singh came to our house and asked us about the details of our family members who are eligible to vote. We showed him the earlier photocopies of the forms-6 and forms-8 along with the signatures of the receiving officials on the acknowledgments. He said that as the addresses were printed wrong, they couldn’t find our names in the area where our votes were allotted, so they cancelled all the four votes and asked us to fill fresh applications in form no-6, along with form-001A. He took the completed forms with two photographs of each of the four of us, and the copies of ration card, as proof of residence. He assured us that this time there will be no mistake as he himself is responsible. He again came back after a few days to verify in person the addresses given on the forms.

The district election office seems to have no copy of the voters list and they said that the party offices are having the copies. When contacted, the party office of the constituency showed us the voters list, which only contained the two names, ie, the names of Dr. Vivian Haque and Navin Haque. The other two names are not included in the list. The district election office has no clue about what happened to the other two votes and asked us to contact, Mr. Kulwant Singh, the Booth Level Officer, in this regard. The mobile phone of the booth level officer is switched off for the last four days, and when finally he came yesterday, i.e. on 11 may 09 he brought only two voter ID cards, of me, Dr. Vivian Haque, and my son, Navin Haque (The addresses printed on the voters ID cards were again wrong, also the names were misspelt). He said that he has no idea about the votes of my wife, Smt. Premlata Haque and my daughter-in-law, Prasannta Haque. He had personally attested and verified the details of our identity and residence before sending the names to be included in the new voters list. What happened to the other two names, he has no idea and does not know who is to be contacted for this discrepancy and lapses.

As tomorrow, i.e. 13 may 09, is the day of polling for Amritsar LS constituency, we are left with no other option than to inform your kind self about this gross negligence on the part of the dealing officials concerned.

Being a senior citizen I am shocked to see the negligent behavior of the officials by preparing wrong documents and omitting the names of my family members in the voting list, thus depriving us of our constitutional right, i.e the right to vote. I am hoping that your office will take a note of this problem of ours and set the things in order immediately upon the receipt of our complaint.
