New Voter ID Card

Dear Sir,

I have already submitted the form no.6 duly filled in and along with the Photo and Ration card copy for addition the name of self and my wife in voter list with Election Commission of India Sector-12 office at Faridabad (Haryana) about 2Months ago but I have not yet received the PHOTO ID Card. Please also inform from where I can the information whether our name have been included in the voter list or not.?
