Sirname different in proofs, problem in getting PAN card

Dear Madam/Sir,

I have following proofs:

for birth: Matriculation Certificate where sirname is Vashisth

and after then (10th std.) I hav been using 'Vashishtha', and so far i didnt faced any problem.

Other proofs:

for address: Bank Statement

others My Degrees and all.

The Representative declined my request on basis of varying Sirname.

Qs: Should an affidavit be an option.?

Qs: Would I face similar problem applying for passport?
