Unable to transfer SB A/C under Senior Citizen Scheme from one post office to another

I am having a SB A/C under Senior Citizen Monthly income scheme (MIS). at Colootola Post Office, which is far away from my residence. I wanted to get it transfered to Naktala Post Office which is near my residence. I have applied for transfer but the Post Master told me that I am to apply in a specific form via Form G. Unfortunately this form is not available in most of the Post Office including GPO and as such i am not able to apply for transfer.

Further I am staying with my son whose office is going to be shifted from Kolkata to Pune soon. So transfer of my account to Pune has become all the most essential.

I request you to kindly look into the matter and guide me as to how I can get this form enabling me to apply for transfer of my account.
