Transaction by use of Debit card reported to be lost / stolen

This is in reference with my Lost Debit Card of HDFC Bank dated January 21,

I reported loss of Debit Card online dated January 23 as branch telephone was not answered on January 21 & January 22 and I was having branch phone nos. only,

There were several transactions on my debit card which was not authorized by me and I notify them to Bank by email dated January 23,

I received response from bank asking to fill Charge Dispute Form and submit to branch (Copy Attached) dated January 31,

There was no response from Bank regarding this matter so again I contact HDFC Customer Care dated February 25 asking for the action taken on my Charge Dispute Form,

On my surprise the Customer Care reply as below:

This is with reference to your query regarding Dispute.

We request you to revert with the following details in order to assist you better:

1. Date of submission

2. Exact Location of the branch where the application was submitted

Please provide us the complete address of branch. Please visit the below link for branch location:

3. Mode through which the application was send (Courier/ Post)

4. Reference number or awb no if sent through courier

So I sent them attached copy of Charge Dispute Form submitted at HDFC Bank, Vejalpur Branch dated January 31 duly acknowledged by bank,

There was no action taken by HDFC Bank in this matter and after several contacts, they were in position to locate the Charge Dispute Form and Allotted the Reference No.: A33610429 Dated March 30,

On April 6 Branch Officer Mr. Biren Shah called me up and ask to submit several documents in response to my Charge Dispute and the list of required documents was delivered to me dated: April 9 (As below):

Subject: Transaction by use of Debit card reported by you to be lost / stolen

We regret to note that your Debit Card has been reported by you to be lost/ stolen/ not received or used. We understand that you intend to dispute subsequent transaction (s) done through the said Debit card.

Accordingly, we request you to provide us the following documents within 21 days of reporting the loss to take this case forward.

FIR (First Information Report) from the concerned Police department

Letter giving details of the event and transactions disputed

Indemnity Letter duly signed as per format and franked for Rs 200/-

Passport copies in case of international transaction disputes

Please note that any acknowledgement or letter in lieu of the FIR will not be accepted.

On submission of the aforesaid documents, we would provisionally credit your account for the disputed amount subject to “hold funds” on such credit.

We will then be investigating the case as per our procedure. Kindly note that you cannot utilize these funds unless the investigation is completed and the findings are in your favor. You are requested to take note of the same before carrying out transactions in the subject account.

All above documents were submitted to Branch dated April 29 (Acknowledge Copy of Documents is Available),

Again they asked for one more documents dated May 06 as per below mail (This was not specified in the list of documents provided dated April 9) :

Dear Mr. Gotecha,

We have already forwarded your request to concern department for early resolution. But concern department had raised following query.

'"If FIR is in any other language other than English/Hindi, a translated copy (Notarized) of the same to be provided".

So we request you to please provide the FIR copy in Hindi so we can proceed your request further.

This copy was submitted to Branch dated May 11 (Acknowledge Copy of Document Available),

There was no communication from Bank after receiving above documents after 3 weeks neither my account is provisionally credited with the amount disputed in my account
