Cheque given to representative, but not yet produced in HDFC

I issued a cheque on 17.02.2009 to HDFC representative Ms. Debastuti (HDFC-SLIC 9831774487). But till now she can't submit my cheque to HDFC office. I tried to contact her, but still now, she refused to say anything about my cheque. Or, not receiving my call. My Policy No. 1149287, Agent nameMr. Kushan Majumdar.
