Heavy leakage of gas from valve

Dated 12.11. while changing the cylider, heavy gas leakage observed from the valve.

Looking at the valve side and after comparision with the other empty cylinder (Same make) there was rubber seal missing in the defective cylinder.

I need the urgent replacement og the defective cylinder & company shall take the appropriate action for the seal missing which can lead to " EXPLODE".
