Activation of account

My demat acct . is an old acct and doesn't have future and option trading enabled. Customer service sent me the contract form on 10th Jan and i submitted the same to icici on 12th Jan. This was submitted in barakhamba road office- first floor of ICICI securities to the manager Mr. Kumar.

I have been running from pillar to post for last 18 days and till today no action has happened on the same.

Twise i registered the complaint with icici customer service. They ask the phone number and say SMS will come for complaint number but that never happens.

Request you to pls help.
