Wrong spelling of Name and date of birth in Election Identity Card

Dear Sir/Madam,

My Voter ID card No. is: PBA5543087

In my voter ID card the spelling of my name and my fathers name has been wrongly written i.e Navin and Omprakash respectively. My accurate name is Naveen Kumar Kadian and my fathers name is Omparkash Kadian. My date of birth is also wrong. My date of birth is 14.09.1984 accurate age on 01-01 is 24 years.

Please issue me a new card or guide me how to get a new one.
