The same mistake is happened with my wife's Identity Card bearing NO: XPD0403477, which needs to be rectified. Following information required to be recified.

Husband's Name: SANDIP CHOWDHURY

Date of Birth : 25.12.1971

Address: S-3, 606, SHALIMER GARDEN, EXTN : 1, SAHIBABAD,

GHAZIABAD, TEHSIL: GHAZIABAD, DIST: GHAZIABAD (UP)

PIN : 201001

Please rectify the mistake and send me the fresh Identity Card to our residencial address.
