Non payment of PLI servival benefit

I have the honour to inform you that, I have not received survival benefit Rs. 4000/- against PLI policy No. AM/AEA/24986-CS till today after getting payment sanction from Dy. Divisional Manager (PLI) Assam Circle, Guwahati-781001 vide No. LI/AM-AEA-24986-CS/DI/86-Krjh dtd. 19.11.2008 from Gossaigaon Post Office PIN 783360, Dist. Kokrajhar, Assam which was due on 21.03.2008.

Kindly look into the matter and take necessary step for payment with interest from your end. your kind action in this regard is highly solicited.
