Not received of PAN

i mr. zahiruddin faruk shaikh staying in dehu road had applied for pan card in the month of october with the application sr. no. 042607074.

i am informing you about not receiving of my pan card.

as it must be delivered with in 15 days from the date of application so i request you please look into the matter and dispatch my pan card as early as possible.
