Complain against faulty motherbord

Respected sir,

i would like to say that my intel motherboard S.No-D945GCNL is not working properly..

there is some problem in the mouse port and in hardisk port or some internal problems because of this my pc is not working. It is still under warranty period.

kindly show me the next way to replace it..
