I have lost my PAN card no: ALUPM 8449E. How to get a new/duplicate PAN Card and what are the exact procedures.

Do I first need to file a FIR?

And after filling FIR where am I supposed to submit it and do I need to pay any fees for the duplicate PAN. If so, how much is the fee for it?
