DEFICIENCYOF SERVICES AND NEGLIGIANCE DUE TO NON PAYMENT OF PENDING DEATH CLAIM OF MY WIFE

Respected sir/Madam

I, the above named is legal heir and nominee of my deceased wife Dr. Reena Menghraj Khushiramani, herby request you that my wife was a policy holder of bearing policy nos.924026574 and 924031822 made in 2006 from Ulhasnagar Branch and polcy no. 947302891 from ITCHALKARANJI branch 94A n district Kolhapur. My wife died on 04/03 and I have submitted all necessary document required by L.I.C.OF INDIA to both the offices. Ulhasnagar branch made the investigation and discharged my claims of both the policies BY CHEQUES ON 20/07 and I have submitted the Xerox copies of both the claim cheques and information lettrs to Kolhapur district office within 2to 3 days for their convinience because ITCHALKARANJI OFFICE FORWARDE the claim to KOLHAPUR DISTRICT OFFICE but kolhaput district office branch no. not given any response and not taken any action regarding the my pending claim. I have written three times request letters to KOLHAPUR DISTRICT OFFICE regarding the status of my pending claim and even after that they have not responded, finally I have posted "RIGHT TO INFORMATION ACT 2005" letter to Kolhapur office and even after that they have not responded, so I have made information from L.I.C. agent Mr. Prakash Panjwani by mobile no. 9422590201, who tld me that Kolhapur office sended three letters to THANE DISTRICT OFFICE TO send them the copy of INVESTIGATION REPORT MADE BY ULHASNAGAR BRANCH but THANE DISTRICT OFFICE have not send any reply to KOLHAPUR DISTRICT OFFICE that is why there is delay

I am requesting you kindly help me by directing the KOLHAPUR DISTRICT OFFICE to discharge my claim as early as possible otherwise they come under S.2 (O) of Consumer Activism 1986 which is deficiency of services

Kindly direct the KOLHAPUR OFFICE TO GIVE ME INFORMATIO REGARDING STATUS OF MY PENDING CLAIM because it is mandatory under "RIGHT TO INFORMATIN ACT 2005"
