I am naman jain c/o 1567, elza school, yadav colony vivekanand ward jabalpur (m.p.) 48 ph.no. 0761-2417939, 4005141, 94253-72403 want to complain against anish nigam, prop. Of Life Graud Health Management. This is health insurance company it sells me health policy on 04/10 but didn’t give me policy bond $ policy card. I requested to him so many times but company’s agent Satya Narayan Patel, he always made promise to do so. In policy my 05 family members are coverd named as-1. Indira Jain (mother) 2. Naman jain (my self) 3. Preeti Jain (wife) 4.Kumar Shoury Jain (son) 5.Kumar veer (son). On 25/04 my wife (Preeti Jain) mat an accident, she admitted in J.K. Hospital from 25/04 to 02/05 & hospital informed to police & did Medical Legel Case (M.L.C.) I paid total hospital bill rs. 24, 135 . I informed to agent (Satya Narayan Patel) but he said company has gone & I can’t do it any thing for you. I checked his office in Jabalpur & Bhopal (Head Office) there is no office their they left out & all phone nos are out of order.
