Money back for 15% SA not recd after 5 years

Dear Sir/Madam,

I've taken one LIC policy bearing no. 252692306 for self. The SA is Rs. 1 lac & DOC is 28th jan. As per the policy, I'm supposed to get 15% survival benefits of SA after 5 years. So far, I've not recd any cheque for Rs. 15000 from LIC for the survival benefits.

Kindly help me to get the survival benefit cheque after 5 years along with interest since the due date for this payment was 28 Jan.
