A application form(BSNL)
To know the Information regarding the reception of a registered form

With due respect I JAGA MOHAN SAHU, send a applicatoon form (BSNL), on Date-24/03, (serial no-3219), from GANGA NAGAR POST OFFICE, Bhubaneswar-751003, Orissa, with charges of 27.00 rupees in the address of

O/o the CGM Telecom, BSNL,

Orissa Telecom Circle,

Bhubaneswar-751 001.

I want to know wheather it is delivered to the above mention address or not? because i don not get any response from the BSNL office regarding my reception of formtill now.

please inform me as soon as possible, so that i will take necessary steps,
