Dear Sir/Madam,

I have applied for 2 PAN cards on 19.04. Acknowledgement slip No is 89041 (Upendra SC) & 89041 (Jyothi G). Till now I didn't received my PAN cards. Further this issue I discussed with Ms. Shwetha, she is telling that it is misplaced & asked me to resubmit the fresh forms. This is not a sufficient answer for my quarry.

Kindle look-in to this as first priority & do the needful.
