Passport no issued

I requested for new booklet of passport in August first week as my previous passport (B1000732) was damaged with file reference Number D002479

Police came to my residence around 4th of oct and report is clear as per passport office site but till date i have not received my passport.

Requesting you to please let me know when will i get my passport.
