Wrong Husband's name, address and date of birth

Dear Sir,

This is with the reference to the Election Identity Card no. LRY5750302, where in my Husband's name is mentioned as Rajesh Pandya instead of Bhoopesh Pandya and my date of birth is mentioned as xx/xx/1981 instead of 04/08/1976.

I Stay in Hyderabad at the given address below and the same is wrongly mentioned on Identity card.

Kindly let me know how can I change the same at the earliest.
