Dear Sir,

With a heavy heart I have to submit before you the following facts for your kind consideration and immediate redressal

I submitted a bill in the name of my wife Mrs. Mousumi Majumdar who had undergone treatment in Ramakrishna Mission Seba Prathisthan, 99, Sarat Bose Road, Kolkata- 700026 during 29th Jan to 5th Feb. 2008 along with all necessary documents. All my efforts to get the payment or a reply has failed. Even when I wanted to talk to the divisional manager, (Division XIV) Mr. Konar, he was so rude over the telephone that dialogue could not be completed.

As the bill has remained pending for nearly six months I would request you to kindly see that I get the payment within the next couple of weeks. Otherwise I will be left with no alternative but to approach the next competent authority including Insurance Ombudsman.
