Name on Voter ID

Sir,

I am to able to use my voter id as ID because NAME written on that VOTER ID is wrong. My name is SANDEEP KHARI but they have written only SANDEEP on that and my house no. is 4 but they have written 214.I request you to please tell me the Procedure to change my name & details on that. I am giving details of my VOTER ID below:

NAME- SANDEEP KHARI

FATHER'S NAME- ILAM SINGH
