PAN card not received

Sir,

I have applied for PAN card on 15th sept'08. vide coupon no.028535111 and CSF no. 704265134.

Hence there was a mismatch in Fathers name as per the letter received vide no. UTITSL/PAN/115566964 /453495 DATED 25.09.08 the proof for the same was submitted on 3rd october'08.

Till date i have not received my PAN card.

Request your goodself to to look into and arrange to release my pan card.
