NSC issued by Post Office
Issue of Duplicate NSC

I have lost NSC for face value of Rs. 10, 000 which was issued in January. In order to complete the formalities for issue of duplicate NSC, an AFFIDAVIT is also required to be given. The post Office authorities are unable to provide the format and hence issuance of duplicate certificate has been delayed by over one year. Despite my visiting the Post Office time and again, the format of Affidavit is not made available.
