Coorrected Pan Card not received Since Years

My Pan Card No. : ABEPR1729C, I am filling I.T. returns at Patan 384265.

I requested rectification of my Grand father's name vide Ack. No.072630100008082 on November 28. My Grand father's name is NANALAL, which was mentioned as MANALAL.

I received a Pan Card with corrected name of my Grandfather but my surname was wrong in the fresh card I received. It was mentioned as RAYAL instead of RAVAL.

Again I requested for the correction in surname vide Ack. No.030010100107525. After sevaral reminders I received a letter from MS NEETA WAGH dated July 16 No. TIN/CRM/SPES058/61773/58574 to submit feedback sheet, which was submitted imidiatley. Since then I am looking for corrected Pan Card. I can't understand the mismanagement at large.
