Pan card
My pan card is defaced, please suggest me how to apply for a duplicate

Hi The pan card that i have is being defaced, is it's topmost plastic cover which shows the pan no, name and other details has come off, and even i tried sticking it again, but it proved to b futile, please suggets me some alternatives and help me in getting a duplicate PAN card. i am waiting for a quick and positive reply from you.
