Eapplication of the pan card which was never recieved

My pan card # is AZBPP6049E, which I have applied around 4 months back. But unfortunately I have never received the card. I got my pan card # from the Income Tax Office in Visakhapatnam also was informed that need to get an allotment letter from another department for the card reapplication.

Please guide me through the correct procedure for the card reapplication, also need to update my address as I recently shifted.

Is there any possibility of getting the allotment letter to my mail Id.

Please help me out with this issue as I require the pan card very urgently.
