Pan card not reveived

I had submitted form for name correction in my PAN card, yet to receive the same. my pan card number is ACXPN6252E. Name shown is Alwarsamy (my father's name) Alwarsamy Naidu; I need to change it to A.K.R.Srinivasan with address change from Nagpur (Maharashtra) to Chennai (Tamil nadu) Hence kindly arrange to issue Pan card after necessary name changes...
