Dear Sir/ Mam,

I am B.Praveen bearing the PAN Card # BDAPB6415J. However, I have not yet received my PAN Card since the time 09 MAR I have applied for the same. I have even sent a couple of hand written letters to the PAN card office, Mumbai as suggested by them through an email requesting for the delivery of my PAN Card. Still, I have not received the PAN Card. This has caused a lot of inconvenience to me to carry out my formalities with my bank.

Kindly do the needful and send the PAN Card ASAP at below mentioned address,
