Regarding PAN Card

Dear Sir,

I have applied for a PAN Card in Jan 08 and have got the acknowledgement; When verified I could get my PAN number as AGHPN5445M. But unfortunately I haven't recieved the card and also my surname on the card has been printed as NAGULA instead of NALLAMREDDY. I have given this PAN number in office for the tax deductions.

So could you please advice whether I need to take a new PAN card or is there any procedure that I can retain my PAN card with modified details.

Kindly request you to provide me certain inputs on this...
