Not receive the PANCARD from UIS

Dear Sir,

I have lost my PANCARD NO.AAWPH9117K and applied for a new card on 17.02.2009 through UTI investors service but till date I am yet to recieve the same for which I am facing trouble in opening of bank accounts and others..So may I know the proceedure to follow to get the same.
