Delay in passport issueing

Sir,

I am Nadella Vamsee Krishna, my passport ref.no is z509286-08.

i have submitted my passport form on 28/8 in Guntur.it's almost been an

year ;yet i did not receive my passport. The status of my ref.no shows a "blank".

I hope you would look into the matter and intimate me at the earliest.
