Poolice enquiry have not recived after 40 days apply

This is a complaint for the issuance of my Passport (File N. LKOA09449209). I had applied my passport on 6th MAY at Passport office Lucknow. My district Azamgarh in first week of May, but my enquiry form was not there till date.
