LOSS OF ATM CARD

Dear Sir/Madam,

I am Lenin Joseph Valiyamattam retired from Indian Navy on 31 Jul. I have an SB account (A/C # 1207010300015918) in PNB, BARC, Cheetah Camp, Mankhurd (Extension counter: CABS). I have lost my ATM card about a couple of months ago. I did not try to block it because it was already de-activated when I was trying to withdraw money from ATM counter (MG Road, Eranakulam, 24 May) using wrong password (Forgot).

Kindly send me a new ATM card to my home address available in bank database or please advice how do I get a new one.
