LOSS OF ATM CARD

Dear Sir/Madam,

I am maneesh vill. kothe manwal khanpur chiwk pathankot punjab. I have an SB account (A/C # 8) in (2917000503195813) PNB, dhangu road pathankot.. I have lost my ATM card about 2 days ago. I did not try to block it because it was already de-activated when I was trying to withdraw money from ATM counter.

Kindly send me a new ATM card to my home address available in bank database or please advice how do I get a new one.and cancel my account
