Dear Sir,

I would like to submit that I had opted for VRS in the year 2003 from Indian Telephone Industires Ltd., and was relieved from the services of the company w.e.f. 30.11.2003. At that time I was less than 50 years in age and was not eligible for monthly pension. Hence, Employee Pension Scheme 1995 Certificate vide No: KN/KRP/4392 was issued in my favour. It was mentioned that this certificate is to be surrendered along with the claim in Form No. 10. D.

After doing all the formalities I was not being paid pension upto June whereas I was due for monthly pension w.e.f. Dec. 2011. I approached all the departments but of no use. In June I got the pension of Rs. 998/- with some arrears in my bank account. I am surprised to see that without informing or sending me any paper or details the concerned deptt. has depositing my monthly pension to my account.

I have been advised by my Ex Head Office, PF commissioner Bangalore to get in touch with your office as they have already forwarded all pension related documents to your office. Also PF Office, GOI has also advised me to contact your office for complete details. As such you are requested to please send my my pension documents so that I am able to quote the reference of my pension case in future and oblige. Thanks and warm regards. BIMLA SATHOO MOB.
